Description 

A 45 year old man presents to the outpatient clinic with a chief complaint of numbness and weakness in right foot for the past 6 months.  Initially had sharp pain in the foot followed by numbness lasting a few weeks.  Weakness occurred 1 month later.  He has well controlled diabetes, as well as hypertension and hyperlipidemia. As a child, he had an upper respiratory tract infection followed by lower extremity weakness X 1 week.
