A 62 year old man with history of sarcoidosis presents with fever, headache, blurry vision x 3 days.
He has a history of:

Intermittent fever x 3 weeks, up to 101 at home -
Cough x 3 months, non-productive -
Worsening memory x 3 months -
Unintentional weight loss >30 kg over the past year
Past history of sarcoidosis, CMV retinitis, bladder cancer and anal cancer.
